We help families apply for government funding and support, financial resources and assistance, fill out or prepare documents connect to community resources and create a circle of care. This is a collaborative, individualized service to help families who need support with advocacy, applications, documentation, and establishing community connections. Information, support and assistance is available to clients and their families to navigate the next phase of their stage of life, including help with: Completing application forms correctly Organizing supporting documents Ensuring timely submission Orientation to financial services in the community Representation for the person with the disability to assist in decision-making, daily activity living, work, volunteer opportunities.
