When should you reach out for Relieve-Me Consulting Services? Do you need support to advocate for yourself, or matters related to your child or loved one's specific diagnosis? Do you need assistance applying for government funding such as ODSP, PASSPORT funding, SSAH, ACSD, Ontario Works etc.? Do you need assistance with creating compelling documents for advocacy purposes? Do you need help with planning, or support to transition to community or adult services? Do you need help liaising with or getting referrals to community and government agencies, programs, or services? Do you need support with building a network of care? If you answered yes to any of these, you need the service of Relieve-Me Family Consulting & Advocacy Service. We help families apply for government funding and support, financial resources and assistance, fill out or prepare documents connect to community resources and create a circle of care. This is a collaborative, individualized service to help families who need support with advocacy, applications, documentation, and establishing community connections. Information, support and assistance is available to clients and their families to navigate the next phase of their stage of life, including help with: Completing application forms correctly Organizing supporting documents Ensuring timely submission Orientation to financial services in the community Representation for the person with the disability to assist in decision-making, daily activity living, work, volunteer opportunities. Let us help relieve the pressure and stress associated with appplying for government funding and filing forms.
Cancellation Policy
We require a minimum of 24 hour notice to cancel a social support booking. If you cancel less than 24 hours prior to the scheduled service date and time, the entire time will be non-refundable.
